Welcome to the Hickam Elementary School Health Room, dedicated to supporting the health and well-being of all our students. Our school health staff works to ensure a safe and healthy learning environment, providing essential health services, managing student health records, and responding to medical needs during the school day. We partner with families to keep our students healthy, engaged, and ready to learn.

For the health and safety of all students and staff, please keep your child home if they exhibit any of the following symptoms:
Fever: 100.4°F (38°C) or higher. Your child must be fever-free for 24 hours without the use of fever-reducing medication (e.g., Tylenol, ibuprofen) before returning to school.
Vomiting or Diarrhea: Your child must be free of vomiting or diarrhea for 24 hours before returning to school.
Rash: Any unexplained rash or rash associated with a fever. A doctor's note may be required to return.
Persistent Cough or Sore Throat: Especially if accompanied by fever or other symptoms. A doctor's note may be required.
Eye Discharge: Redness, itching, and/or discharge from the eyes (potential conjunctivitis/pink eye). A doctor's note and/or 24 hours on medication may be required to return.
Head Lice: Your child must be treated and free of live lice before returning to school. Please notify the health room if your child has lice.
Any Contagious Illness: (e.g., confirmed flu, strep throat, chickenpox, mononucleosis) until a doctor's note clears them to return or a specified symptom-free period has passed.
Please notify the school office and/or the Health Room if your child will be absent due to illness.
For the safety of all students, specific procedures and required forms must be followed for any medication (prescription or over-the-counter) to be administered during school hours.
SH36 - Request to Store and Administer Medications (rev. 2/2024): This is the primary form required for all medications (including those for anaphylaxis, asthma, seizures, and other daily medications) to be stored and administered at school. It must be completed by a physician and signed by a parent/guardian.
SH36 DM - Request for Diabetes Care and Medication Administration (rev. 8/2024): This specific form is required for medication and care related to diabetes management at school.
Student Medication Self-Administration and Carry Authorization (rev. 1/2024): This form is required to allow students to self-administer specific emergency medications such as an albuterol inhaler, epinephrine auto-injector, and/or glucagon.
Volunteer Authorization to Administer Epinephrine and Glucagon (rev. 6/2023): This form allows authorized volunteer DOE employees to administer epinephrine auto-injector and/or glucagon in emergency situations.
All medication must be signed in by a parent/guardian to the nurse's office/health room.
Medications may NEVER be transported to or from school by a student.
If the medication is a prescription, it must come in the original, unopened pharmacy-labeled packaging with the student's name, medication name, dosage, and administration instructions clearly visible.
If the medication is an over-the-counter medication prescribed by a doctor, it must come new/unopened and labeled with the student's name, date of purchase, and reason for administration.
Expiration Dates: Please ensure all medications are within their expiration date. Expired medications cannot be administered.
Changes in Medication: Any changes to medication (dosage, time, discontinuation) require a new physician's order.
End of School Year: All unused medication must be picked up by a parent/guardian by the last day of school. Medications not picked up will be disposed of according to district policy.
Questions & Assistance: For questions and assistance with medication forms, please contact your child's school or the Hawaii Keiki Health Hotline at 844-436-3888, available Monday through Friday, excluding HIDOE holidays and breaks.
If your child has a chronic health condition or a medical condition that might require emergency care, please inform the Health Room staff immediately.
Asthma: Students with asthma should have an up-to-date Asthma Action Plan on file in the Health Room, in conjunction with the required SH36 form for medication administration.
Severe Allergies (Anaphylaxis): Students with severe allergies should have a Food Allergy & Anaphylaxis Emergency Care Plan (or similar) and prescribed EpiPen(s) on file in the Health Room, along with the necessary SH36 form.
Hickam Elementary is a Nut-Free School: To ensure the safety of students with severe allergies, Hickam Elementary School is a nut-free environment. We kindly request that families do not send any products containing peanuts or tree nuts (such as almonds, walnuts, cashews, etc.) to school in lunches or snacks. Your cooperation helps protect our students.
Diabetes: Students with diabetes require a detailed Diabetes Medical Management Plan (DMMP) developed by their physician, along with the specific SH36 DM form for diabetes care and medication administration.
Seizures: Students with a seizure disorder should have a Seizure Action Plan on file to guide staff on how to respond during a seizure, supported by the necessary SH36 form if medication is involved.
Other Conditions: Please speak with the Health Room staff about any other chronic health conditions, medical needs, or disabilities that may require support or accommodation during the school day.
Individualized Health Care Plans (IHCPs): Our health staff can work with you and your child's physician to develop an IHCP to ensure their needs are safely met at school.
Emergency Contact Information: Please ensure all emergency contact information on file with the school is accurate and up-to-date. In case of a medical emergency, we will contact parents/guardians first. If unable to reach them, emergency contacts or emergency services will be contacted as necessary.
Hawaiʻi State Law mandates that all students meet specific physical examination, immunization, and tuberculosis clearance criteria before attending any childcare facility, preschool, or public/private school in the state. Hickam Elementary School collaborates with the Hawaiʻi Department of Education (DOE) and Department of Health to ensure students fulfill these essential health prerequisites.
This is the official health form required for your child's doctor to complete after their medical physical and immunizations.
Must be completed within one year before:
The first date of attendance at a childcare facility, preschool, or school in Hawaiʻi; AND
The first date of attendance in the seventh grade.
Must be performed by a U.S. licensed physician, APRN (Advanced Practice Registered Nurse), or PA (Physician Assistant).
Immunizations are required for childcare facility and school attendance.
Required immunizations depend on the age of the child (for childcare or preschool) or the grade of the student.
All immunizations must meet minimum age and interval requirements between vaccine doses.

TB clearance received from 12 months of age or older will be accepted for School Entry Requirement for all students DOE Pre-K through 12.
May accept TB Blood test (QuantiFERON, T-spot) for clearance for students 5 years of age or older.
Temporary Chest X-ray clearance must have a follow-up TB Skin Test by the expiration of the Chest X-ray date, or the student should be excluded from school.
All required health information must be properly documented and signed by a U.S. licensed practitioner (MD, DO, APRN, or PA).
If a student lacks documentation of all required immunizations or a physical examination, they may be granted provisional entry if they verify an upcoming medical appointment to complete the requirements.
Students may be exempt from immunization requirements for medical or religious reasons, provided they present the appropriate documentation to the school.
